Avatar user support and resources
Information, resources, and support for SFDPH Behavioral Health Services (BHS) Avatar system users.
About Avatar
Behavioral Health Services uses Avatar, an electronic health record (EHR), to enter clinical and service information such as assessments, treatment plans, progress notes, and medications. In addition, Avatar captures client health insurance information required to produce claims to Medicare, Medi-Cal, and other insurers, and to support managed care functions.
Technical Support
If you're an Avatar user, please contact the Avatar Help Desk at 628-217-5196 from 8:00 am to 5:00 pm Monday-Friday or email Avatarhelp@sfdph.org.

Training
Beginning December 2025, Avatar training will no longer be conducted by a trainer. Instead, all new Avatar users (and users who are reactivating an account that has been inactive for more than one year) will need to watch a video online and then complete and submit an attestation of completion.
After submitting an Avatar Account Request Form, the Accounts Team will email the user with information about next steps, including a link to the training video and a copy of the attestation form to be submitted at the conclusion of training.
As always, new users must enlist the assistance of their colleagues and supervisors for support with the Avatar workflow that is specific to their agency and job function.
Avatar account forms
Complete and submit an Avatar Account Request Form to initiate the creation of a new Avatar account, update an existing account at a new program or with new credentials, or reactivate an account that has been inactive for more than one year.
