Thank you for your Displaced Tenant Housing Preference certificate application for fire displacement

We have received your application for a Displaced Tenant Housing Preference certificate application for fire displacement. To finish applying, you must upload, fax, mail or drop off your government issued ID and required documents.

Complete the Fire Displacement Verification form

For displacement due to fire, you can download the Fire Displacement Verification form which must be completed by a public safety official (if you cannot download the form, the official can provide one).

Collect one of the following three options (A, B, or C) to prove residency at the time of displacement

Option A - Copy of signed lease at displacement address

OR

Option B - One of the following utility bill documents showing the service address (must be dated within 45 days prior to the date of the fire displacement):

  • Gas and electric
  • Water
  • Garbage
  • Cable and internet
  • Land line phone

OR

Option C - Two of the following documents (must be dated within 45 days prior to the date of the fire displacement):

  • ID: Government-issued ID with displacement address
  • Utility Bill: cellular phone
  • Government: Public benefits records (e.g. SSI/SSDI, Medi-Cal, General Assistance, CalFresh), IRS/FTB tax returns, voter registration, or vehicle registration
  • Employment/School: Employment documents, pay stubs, school records (with date of birth), or proof of resident tuition
  • Medical: billing documents
  • Financial: Bank/credit union statements, credit card statements
  • Insurance: Medical, dental, vision, life, home, rental, unemployment insurance, or vehicle insurance documents

Complete the Declaration of Continuous Occupancy form

For displacement due to fire, you must complete and sign the Declaration of Continuous Occupancy form.

Label your documents

Please make sure documents you submit to MOHCD are clearly labeled with your name and the Displaced Tenant Housing Preference certificate application number you received in your confirmation email.
