POLICY: All childcare personnel are required to have on file a current tuberculosis (TB) screening assessment signed by a licensed health care provider, and proof of current immunization status, prior to working with the children. All childcare Center personnel must also have a health assessment (LIC 503) signed by a licensed health care provider (not required for family child care homes).
Immunizations must be current according to the latest state health department standards.
A TB re-assessment is required every 4 years, or sooner if staff develop risk factors for TB.
The Child Care Center requires TB re-assessment every 4 years, or sooner if staff develop risk factors for TB.
PURPOSE: To assure that the individual can sustain personal health during employment.
To protect the health of the children.
To satisfy Licensing Regulations.
PROCEDURE:
- Staff member’s health file will be kept and accessed by the Director only.
- Staff member’s health file shall contain:
- Health Screening Report (LIC 503)
- Immunization Record
- TB Form - California School Employee Tuberculosis Risk Assessment Questionnaire for Pre-K, K-12 schools, and community college employees, volunteers and contractors (see J-4)
- Injury and Illness Data
- Current Credentials
