Eligibility and referral process for assisted outpatient treatment (AOT)
Program criteria to qualify for assisted outpatient treatment
Call 911, if you or someone else is in immediate danger of harming themselves or others.

A person can join the program if all of these are true:
- They are 18 years old or older.
- They live in San Francisco.
- They were offered mental health help before but said no.
- They need support to stay safe in the community.
Additionally met these requirements in one of two ways:
Option 1: Hospital or Jail Visits
- In the last 3 years, they were in a mental health hospital or jail two different times because of their mental health.
- One stay in a hospital and one stay in jail also counts.
Option 2: Serious or Violent Incident
- In the last 4 years, they did something very serious or violent because of their mental health, and it caused them to:
- Go to jail, or
- Stay in a mental health hospital.
Select the type of referral needed, and submit to aot-sf@sfdph.org.
Family/Community referral
Family/Community referrals are to be used by parents, siblings, child, or roommates.

Provider referral
Provider referrals are to used by people working with individual such as; license mental health providers, director of mental health institution or individual, probation and parole officer, or a judge.
