Eligibility and referral process

Program criteria to qualify for assisted outpatient treatment

Call 911, if you or someone else is in immediate danger of harming themselves or others.

To qualify for AOT program a person must:

  • Be 18 years old and over.
  • A San Francisco city resident.
  • In the past, they have said no to help, even when they were offered treatment.
  • They are unlikely to safely survive in the community without help.
  • The person has been in and out of jail and or the hospital for mental health problems in last 3 years.

And one of the following situations:

  • One or two inpatient psychiatric hospitalizations and incarcerations due to mental health disorder within the last 3 years.

or

  • One act of serious, violent behavior towards oneself or others due to their mental health disorder in the last 4 years, resulting in incarceration or inpatient psychiatric stay.

Select the type of referral needed, and submit to aot-sf@sfdph.org.

Family/Community referral

Family/Community referrals are to be used by parents, siblings, child, or roommates.

Provider referrals are to used by people working with individual such as; license mental health providers, director of mental health institution or individual, probation and parole officer, or a judge.
