提供者目錄可協助會員尋找有關我們計劃的網路內提供者的資訊。
格式
供應商名錄可依要求免費提供紙本版，五個工作天內即可收到。經您許可，我們也可以透過電子郵件發送電子版。請將請求發送至 BHSMemberServices@sfdph.org。
如有需要，我們還可免費為您提供輔助工具和服務，包括但不限於大字印刷文件和替代格式文件。請致電 1-888-246-3333 或 1-415-255-3737（TDD：711）。
舊金山行為健康服務中心 (SFBHS) 每月審核服務提供者名錄。服務提供者通知 SFBHS 後，名錄將在 30 個日曆日內更新。
