Mental Health and Substance Use Disorder Provider Directories
The provider directories help members find information about our plan's in-network providers.
Formats
Provider directories are available in paper form without charge upon request within five business days. They can also be sent by email in electronic format, with your permission. Send requests to BHSMemberServices@sfdph.org.
Auxiliary aids and services, including but not limited to large print documents and alternative formats, are also available to you free of charge upon request. Call 1-888-246-3333 or 1-415-255-3737 (TDD: 711).
San Francisco Behavioral Health Services (SFBHS) reviews the provider directory monthly. Updates are made within 30 calendar days of provider notification to SFBHS.

*DMC-ODS stands for Drug-Medi-Cal Organized Delivery System.
