The Program Monitoring Visit
- The Program Monitoring Visit - After the end of the contract year a BOCC program monitor will reach out to the program to schedule the monitoring visit.
- How to Prepare - The Program Declaration of Compliance (PDC) that was sent to the program at the beginning of the year spells out all the materials you should have ready at the time of the visit. The PDC for your program can be found here: BOCC Forms, Policies, and Training Materials

Program Monitoring Report
After the Program Monitoring Visit:
The data gathered at the monitoring visit and from the various data sources are captured in the Program Monitoring Report. The Program Monitoring report shows program performance for four areas:
- Program Performance - contracted performance objectives
- Program Deliverables - contracted units of service/unduplicated clients
- Program Compliance - local, State, Federal, and Grantor requirements as detailed in the BOCC Program Declaration of Compliance found here BOCC Forms and Policy Documents
- Client Satisfaction - surveying clients to measure satisfaction with services
- The results of the four domains produce the Overall Program Rating for the given monitoring period
Program Monitoring Scoring
The scoring of the Program Monitoring Report varies by DPH section, depending on the requirements of their work. Details on the scoring can be found here:
