Submit a Quarterly Report

Learn how to submit updates on your grant project activities.

Community Challenge Grants Program

Grantees must send us a report every three months. Your report is due 30 days after the period ends.

  • Report Period: January 1 - March 31. Report Due: April 30
  • Report Period: April 1 - June 30. Report Due: July 31
  • Report Period: July 1 - September 30. Report Due: October 31
  • Report Period: October 1 - December 31. Report Due: January 31

Follow the steps below to submit your report.

Draft Your Report

Use the report template we emailed you to draft your responses. This includes your performance measures and targets. Email ccg@sfgov.org if you need a copy of your custom report template.

Update Your Work Plan

Your custom work plan lists your project's tasks. Email ccg@sfgov.org if you need a copy. For each activity during this period, write one to two short sentences about what you did. Explain any delays.

Take Pictures

Include before and after photos. Show key steps in your project. Make sure you have permission to use the photos, including sign-off from parents of photos of youth. We may use the photos for social media.

If this is the final report, include a photo of your project’s approved signage, which must include CCG's logo.

Get the Submission Link

After each report period, we will email a link to the primary contact. Email ccg@sfgov.org if you did not receive the link.

Complete the Online Form

Some fields will already be filled in, like your contract ID and the report period. Do not change this information.

Fill in the answers to the questions, upload your updated work plan, upload photos, add captions and photo credits.

Email Any Report Revisions

We will review your report. We may ask for more details. Email any updates to ccg@sfgov.org.
