No electronic submissions

We do not accept emailed or faxed claim forms. They must be mailed or hand- delivered.

Submission deadlines

File claims for personal injury, personal property damage, death or loss, or damage to crops within 6 months of the incident.

File tax refund claims within the time listed on the claim form and required by law.

File all other claims, including claims for damage to real property, within 1 year of the incident.

Next steps

Allow up to 45 days for the Claims Division to process and investigate your claim. Any request for information is not an admission of liability or a commitment to pay your claim.

1. Confirmation.

We will send you a letter with your claim number once we begin processing your claim. Use this claim number when asking about your claim.

2. Investigation.

We will investigate your claim.

3. Results.

You will receive one of the following responses to your claim:

(A) Notice that the claim is late or time-barred

This will include instructions with your options.

(B) A request for more information due to missing details

(C) Postponement

We may wait to respond until you finish medical treatment or send us your bills, wage loss forms, and payroll records.

(D) A formal denial

This means we have determined the City is not at fault and will not pay your claim. You can speak with the representative handling your claim.

We will provide your deadline to sue the City if you choose to do so.

(E) A settlement offer

We do not have to respond if your claim needed to be filed within 1 year of the incident and you did not file in time.

If your claim is timely and we do not deny your claim, you have 2 years from the date of the incident to settle or file a lawsuit against the City.
