You can view most of your medical records in your MyChart patient portal without having to make a request.

1. Stop by our office and fill out the following form:

• Chinese Version
• Russian Version
• Vietnamese Version
• Tagalog Version

2. Bring the completed form and a valid photo ID to:

2. Submit completed form along with ID and
