Submit your final report
All FY26 GFTA grantees must submit a final report covering the period from July 1, 2025 to June 30, 2026 to be eligible to apply for FY27-FY28 GFTA funding. Reports must be submitted online using your unique final reporting form link.

GFTA is accepting final reports on a rolling basis until Tuesday, June 30, 2026 at 11:59PM PST. We recommend submitting your final report before April 2026 to ensure your eligibility for FY27-FY28 GFTA funding and to leave ample time to complete the application, which will open in May 2026.
We understand you might have programming and income/expenses past April since the fiscal year ends June 30, 2026; you may forecast your numbers based on the previous fiscal year and/or current data trends.

What to do
1. Use a form template to draft your answers
Grantees are encouraged to prepare responses in advance using one of the templates below:
- If your FY26 GFTA grant amount is under $200,000, use this final report form template: Link to template for orgs that received under $200,000 in FY26.
- If your FY26 GFTA grant amount is over $200,000, use this final report form template: Link to template for orgs that received over $200,000 in FY26
Please follow these template instructions:
- Once the document is open, click “File”
- Select “Download”
- Select “Microsoft Word”
- Once you’ve finished drafting your answers, access the online form using your unique link and copy and paste your answers.
2. Access your unique final report form
GFTA will email all grantees a unique link to the online form on January 30, 2026. Please ONLY use this link to access and submit the form – it will streamline the process, as it has been partially pre-filled with your organization’s information.
For optimal functionality of the form, we recommend using Google Chrome.
Late Reporting Policy
Late Reporting Policy: All reports are due on the date outlined in your grant agreement. Any reports not received are considered late and the grantee is not in good standing with GFTA. We have the authority to pause payments and/or processing on any grants for which reporting is incomplete or outstanding; as well as pause payments on any other open grants or pending new grants or amendments, including fiscally sponsored grants.
