CareLink is a provider portal to limited DPH EHR data. Referring providers and other community organizations use SFDPH CareLink for information access, order entry, and more for shared patients.

Legal authority
We can't grant access to DPH systems (and PHI) without proper legal authority

CareLink site
A site represents your workplace and must exist before you can add user accounts (office, nursing home, Program, etc.).
- Get a new CareLink Site for your group

If your location has a CareLink Site:
- Add a new user to an existing CareLink Site
- Change or Reset your DPH Password
