A site represents your workplace and must exist before your group can request user accounts
Department of Public HealthDoes someone at your location already have CareLink?
Yes: The CareLink site administrator or DPH manager for your location or program can arrange access for you. If you aren't sure who that is we can help.
No: Go to next question
Does your workgroup already have a CareLink site?
Yes: The CareLink site administrator or DPH manager for your location or program can arrange access for you.
Maybe: If you aren’t sure, check current SFDPH CareLink Sites
No: Go to next question
Is this for a DPH or BHS managed department, clinic or program?
Yes: Request a new CareLink site for DPH workforce*
*Workforce, as defined by HIPAA: Workforce means employees, volunteers, trainees, and other persons whose conduct, in the performance of work for a covered entity or business associate, is under the direct control of such covered entity or business associate, whether or not they are paid by the covered entity or business associate.

Do you have the necessary legal agreement(s) in place, and need a CareLink site for a new group's access?
Yes:
- Locate the related contract, SAA or MOU
- Request a new CareLink site for your group
No: Return to CareLink agreement
