We have received your application for a Displaced Tenant Housing Preference certificate application for DTHP Ellis Act, Owner Move-In (OMI), or Illegal Unit Evictions. To finish applying, you must upload, fax, mail or drop off your government issued ID and required documents.

Collect your required documents

For DTHP Ellis Act or Owner Move-In (OMI), or Illegal Unit Evictions, if your name is on the Notice of Intent to Withdraw (Ellis Act) or Eviction Notice (OMI or Illegal Unit) submit a copy of your government issued ID.

If your name does not appear on either of those notices, submit a copy of your government issued ID and any two of the following:

  • Eviction Notice or Notice of termination of tenancy
  • Filed Notice of Intent to Withdraw
  • Motion or order by the Planning Commission (unlawful residential units)
  • Utility bill (landline phone, cable, internet, water, gas, electric, or garbage)
  • Paystub
  • Public benefits records (SSI/SSDI, Medi-Cal, General Assistance, Unemployment Insurance, CalFresh, etc.)
  • School records

Complete the Declaration of Continuous Occupancy form

Complete and sign the Declaration of Continuous Occupancy form.

Label your documents

Please make sure documents you submit to MOHCD are clearly labeled with your name and the Displaced Tenant Housing Preference certificate application number you received in your confirmation email.
