We have received your application for a Displaced Tenant Housing Preference certificate application for rent burden. To finish applying, you must upload, fax, mail or drop off your government issued ID and required documents.

Collect your required documents

In addition to a government issued ID, please submit the documents described below depending on your situation.

If you pay more than half of your income on housing costs, you will need to submit a copy of your current lease agreement.

If you live in public housing or Section 8 housing in San Francisco, you will need to submit:

  1. A copy of your current lease agreement
  2. Proof of last month's rent with one of these documents
    1. Money order
    2. Cancelled check
    3. Debit from your bank account
    4. Screenshot of your online payment

Label your documents

Please make sure documents you submit to MOHCD are clearly labeled with your name and the Displaced Tenant Housing Preference certificate application number you received in your confirmation email.
