Phase One Results

SF Voices Pilot Results

The Office of Health Equity (OHE) has completed Phase 1 of the SF Voices project, which was conducted as a pilot initiative to evaluate the survey platform, establish operational protocols, and identify key populations for engagement. The pilot was carried out during the fiscal year 2024-2025, and a total of 103 responses were collected from participants


Four key findings identified:

  • Significant rates of housing insecurity and unemployment
    • 33% of individuals lack permanent housing
    • 37% are currently unemployed
  • Barriers to healthcare access are widespread
    • 58% of respondents needed a doctor in the past year but didn't go due to cost, clinic hours, or other obstacles.
  • Trans community experiences significant health disparities
    • 60% needed medical care but did not seek it
    • 21 repeated hospitalizations, with 10% due to suicide attempts
  • Emergency services more frequently used than preventive care
    • 51 hospitalizations reported, primarily due to asthma, depression, high blood pressure, heart failure, and overdose

SF Voices Phase II: Next Steps

  1. OHE, in collaboration with the Population Health Division, will conduct community feedback sessions regarding the SF Voices Pilot findings and the Community Health Assessment (CHA). 
  2. Enhance the community engagement process by leveraging pilot insights to expand participant responses.
  3. Employ the San Francisco Community Health Improvement Plan (CHIP) process to establish continuous feedback loops with the community and residents.
