How to Find Your Immunization Records

Guide and resource to locate your immunization records

SFDPH Immunization Program does not collect or archive immunization records. If you receive care through the San Francisco Health Network, you can get a copy of your records online through your MyChart, in person, by mail, email or fax.

Where to start:

  • Digital Vaccine Record Portal: if you received vaccines in California, you can access a copy of your vaccine record from the California Immunization Registry (CAIR). Note: Your digital vaccine record may not be complete if some doses were not reported to CAIR, especially vaccines given before 2000 or outside California, and you may need to contact another state's immunization registry for any out-of-state records.
  • Contact the doctor's office, clinic or pharmacy where your vaccines were given.
  • Contact family members, such as your parents or grandparents, to see if they kept or have pictures of your vaccine records. Search through old papers, baby books, or school forms.
  • Contact the childcare, schools, and colleges you attended.
  • Contact previous employers that required vaccine records.

If you are still unable to locate your immunization records:

Talk to your doctor or licensed healthcare provider about other options, including vaccination or blood tests to check for immunity, that may work for your health and documentation needs.
