Is SFDPH Carelink the right option for your organizations needs?

Does someone at your location already have CareLink?

Yes: The CareLink site administrator or DPH manager for your location or program can arrange access for you. 

No: Go to next question

Does your organization have an Electronic Health Record (EHR)?

Yes: It's always easier to work within your own system than to login to a different system. Work with your system vendor or IT Department to see if you can get what you need pulled into your organizations EHR.

Will you need specific records or specific dates?

Yes: ROI requests should be submitted through HIM.  High volume requesters should work with HIM to set up electronic delivery of ROI (via CareLink).  Learn more

No: Go to next question

Have your IT dept and DPH business sponsor both confirmed that CareLink is the best option to meet your needs?

Yes: Get the necessary legal agreement(s) for access
