Request immunization materials

San Francisco agencies can request immunization materials such as yellow cards or blue cards to be delivered by mail. Some materials are only available by download only. Please allow 7-10 business days for order processing. 

To request materials:

Email immunization@sfdph.org and include the following information:

1. Agency name
2. Agency type (Clinic, School/Childcare, Business, Long Term Care Facility, Other - specify)
3. Contact name
4. Address 
5. Email
6. Phone number
7. Type of material and quantity requested

There is no cost for this service.  However, all orders must have a valid San Francisco address.
