For a City employee that is sponsoring a non-City person working for DPH.

Community Based Organizations (CBO) staff

For a person providing Behavioral Health Services through a CBO in partnership with DPH.

University of California, San Francisco (UCSF) affiliated with ZSFG

For a person in UCSF providing services only through ZSFG locations.

Behavioral Health Services (BHS) using Avatar

For a person authorized to access Avatar for BHS through DPH.

City and County of San Francisco (CCSF) employees

For a CCSF supervisor submitting a request for an employee authorized to access DPH systems
