Get a Respirator Medical Screening and Clearance
Department of Public Health (DPH) Employees
Occupational Health Services (OHS) offers Respirator Medical Clearance screenings for all City and County of San Francisco (CCSF) employees.
If you need a Respirator Medical Clearance screening, call us to schedule an appointment.
If you were previously cleared by OHS for N95 respirator use but have new medical conditions, weight loss or gain, facial surgeries, or dental work, call us to schedule a new evaluation.

Students, Interns, and Volunteers
You must be medically cleared by your primary care physician before you can be fitted for a N95 respirator.
Complete the N95 Medical Questionnaire form and have your provider complete the Respirator Medical Clearance Certificate. Once completed, call us to schedule a N95 Fit Test appointment.

FAQ
1. Can I get my N95 Fit Test done at ZSFG OHS?
OHS provides Fit Testing for all DPH and UCSF employees, interns, and volunteers working at ZSFG.
You can schedule an appointment, drop-in during our drop-in hours, or get fitted with our N95 Team during a scheduled department fit testing session.
2. What kind of Fit Testing do you do?
OHS provides Qualitative Fit Testing.
3. I am unable to tolerate the Qualitative Fit Test. Do you have other testing options?
Other testing options are available through Environmental Health and Safety (EH&S). Call or email us for more information.
4. I have facial hair. Am I able to be fitted for an N95 respirator?
There are a few facial hairstyles that are acceptable with fit testing. For any style, facial hair cannot cross under the respirator sealing surface.
5. I am unable to shave my facial hair. What do I do?
If you are unable to shave and wear a respirator, you are not allowed to work with isolation precaution patients or in situations that require a N95 respirator. Reach out to your manager and DPH Reasonable Accommodations for further assistance.
