2025 ZSFG Employee Flu Vaccine Clinic is now open
Occupational Health Services (OHS) at Zuckerberg San Francisco General Hospital (ZSFG)Flu vaccines are now available for all ZSFG and UCSF staff.
The ZSFG Occupational Health Services Employee Flu Vaccine Clinic will be providing onsite Flu vaccines for ZSFG and UCSF staff from Monday, September 15, 2025 through Friday, November 21, 2025.

Flu Declination:
ZSFG Occupational Health Services needs to have a declination record on file. You may submit a Flu declination using the 2025-2026 Flu Vaccine Declination and Proof of Vaccine Submission Form (ZSFG staff only).
To Submit Proof Of Vaccination:
You can submit your proof of vaccination by:
