A health and wellness center that provides Primary Care medical services, behavioral health care, and wellness programs for people living in the Hope SF Sunnydale community. We are a collaboration comprised of The San Francisco Department of Public Health, Urban Services YMCA and Instituto Familiar de la Raza's SPARK program.

The Sunnydale Community Wellness Center is one of four clinics that provides supports the wellbeing of people living in Hope SF Communities (Alice Griffith, Hunters View, Sunnydale, Potrero Hill).
Services include:
- Physical activity classes
- Wellness activities
- Community events
- Case management (assessing needs, coordinating care, and connecting to other services)
- Support groups
- Individual and group therapy
- Crisis services
- Referrals to primary care
- Chronic disease and acute illness management
- Preventative and sexual healthcare
- Medical Assessments and screenings
- Health education and promotion
