Citywide Case Management
Citywide offer 2 types of case management services for SFAPD clients:
- Clinical Case Management (CCM) services are for clients with serious mental health issues
- Reentry Case Management (RCM) services are for people facing other barriers who need help planning their reentry journey

SEOP Case Management
SEOP provides in custody and community case management services to SFAPD clients who are 35 years of age or older. Clients can get help accessing services they need to support their reentry.
