Reentry case management

We help people with case management as they leave the justice system and reenter the community.

How we can help

Some people need more hands-on support when they leave the justice system.

We can help through case management services.

This may look like help with:

  • Reentry planning
  • Mental health needs
  • Other challenges keeping someone from thriving in the community

Our programs

Read our Catalog of Reentry Services to learn more about our case management programs.
