How to Get Home Health Services at Health at Home

Eligibility criteria for home health services.

Who We Serve

In order to receive services from Health at Home, you must live in San Francisco, and you must meet certain criteria set up by the Centers for Medicare & Medicaid Services (CMS).

All patients must meet the following:

  1. You must be homebound*.
  2. You must need Skilled Nursing or Physical Therapy.
  3. You must meet medical need for intermittent home health services.

*A person is considered homebound if both of the following two requirements are met:

One (One of these must be met):

  • Because of illness or injury, need the aid of supportive devices such as crutches, canes, wheelchairs, and walkers; the use of special transportation; or the assistance of another person to leave their place of residence.
  • Have a condition such that leaving their home is medically contraindicated.

Two (Both of these must be met):

  • There must exist a normal inability to leave home.
  • Leaving home must require a considerable and taxing effort.

How can I get services from Health at Home?

If you or someone you care for needs home health services, you may ask your doctor or healthcare provider to send us a home health referral.

If your provider is within the San Francisco Health Network, they may use the network electronic health record system (Epic) to send the referral. If you have MyChart from the San Francisco Department of Public health, then your provider may be able to send the referral directly within the system.

If your provider is outside of our network, then they may send the referral to our intake fax line at 628-754-4765. You may also ask your provider to give us a call if they have any questions at 628-754-4700.
