Initial Grant Term: 11/1/2025 - 6/30/2027 with options to extend up to 3 additional years
Total Funding Available: $2,917,695 per year

- Submit online cover sheet at
- Attach all other documents to one email and send it to CommDevRFP@sfgov.org. Subject Line should state “Housing Place Based Services RFP Final Proposal” and include your agency name.
- All required documents must be submitted before 5PM on August 28, 2026. Proposals received after 5pm on the due date, incomplete and/or partial submissions will not be considered.
- Please send proposals early ensure the ability to address any unforeseen technical difficulties.

RFP Questions and Responses
Please submit your questions about the RFP in writing to CommDevRFP@sfgov.org by 5PM on August 7, 2026. We will do our best to post our response to your questions by August 13, 2026 on the MOHCD website below.
