Processing time is two weeks and cards will need to be picked up in person.

What to do

Steps to apply for a medical marijuana ID card in person:

  1. Download and fill out the medical marijuana ID card application.
  2. Bring the original letter from your doctor that says you have a medical condition that makes medical marijuana helpful for you.
  3. Bring proof of who you are (like a government issue ID, driver’s license or passport).
  4. Bring proof that you live in San Francisco (like a utility bill, photo ID with your address, or a recent rent or mortgage payment). A P.O. Box address is not acceptable for residency verification.

Caregivers:

  1. If you want to choose a caregiver, you must write their name on the application form.
  2. A caregiver must already help you with things like housing, health, or safety before you apply.
  3. You can only choose one caregiver.
  4. The caregiver must be at least 18 years old.
  5. Your caregiver must come with you when you apply.
