Referral & Navigation Process
You can be referred by your PCP (Primary Care Provider) to GHSF for Navigation-Only referrals.
Referrals and prior authorizations for gender affirming surgeries or procedures to surgeons’ practices must be completed by your PCP and/or Primary Care clinic directly.
