About us

Assisted Outpatient Treatment (AOT) helps people who have serious mental health challenges. It supports those who are not getting help, whose condition is getting worse, and who have had trouble following treatment in the past.

Who this program is for:

  • They are 18 years old or older and live in San Francisco.
  • In the past, they have said no to help, even when they were offered treatment.
  • They are unlikely to safely survive in the community without help.
  • The person has been in and out of jail and or the hospital for mental health problems in last 3 years.

How to refer:

  • Fill out form.
  • Submit by email: aot-sf@sfdph.org or fax: 628-217-7515.

We will review the referral and call you.
