Information, resources, and support for SFDPH Behavioral Health Services (BHS) Avatar system users.

Behavioral Health Services (BHS) uses Avatar, an electronic health record (EHR), to enter clinical and service information such as assessments, treatment plans, progress notes, and medications in a client's record. In addition, Avatar captures client health insurance information required to produce claims to Medicare, Medi-Cal and other insurers, and to support managed care functions.

Training schedules

Avatar training is conducted remotely using Webex; a link to the training will be sent to all registered attendees the day prior to the scheduled class. Please ensure that the attendee's email address is correct and complete when registering, and have Webex installed prior to class time. 

Avatar account forms

Complete and submit an Avatar Account Request Form to initiate the creation of a new Avatar account, update an existing account at a new program or with new credentials, or reactivate an account that has been inactive for more than one year.

Documents

Behavioral health clients who are 18 years or older can use the Avatar patient portal to access their health records. Log in to the Avatar patient portal.

 If you are a client and need help accessing or navigating the portal, call the Avatar patient portal help desk at 855-224-7782, Monday-Friday, 9:00 am to 5:00 pm.

Note: For mental health clients, records up to May 21, 2024, are potentially available through the Avatar portal. Mental health records for services provided on or after May 22, 2024, are documented in Epic, not Avatar, and are available through the MyChart patient portal. For more information, visit MyChart.
