Street Team Consolidation
As of July 1, 2026, City street teams operate under the Department of Public Health. This new phase strengthens the city’s broader work to improve street conditions by more directly integrating street-level outreach with the health and clinical systems that drive long-term outcomes: treatment, medication management, stabilization, and ongoing care coordination.What we do
Medical services
We provide health care that includes:
- Urgent Care
- Primary Care
- Wound care
- Support for ongoing health conditions
- Testing and treatment for HIV, Hepatitis C, and other infections
- Linkage to ongoing medical care
Mental health services
We offer mental health services including:
- Case management
- Mental health support
- Connections to mental health treatment
- Psychiatric care
Substance use services
We provide support for people struggling with drugs or alcohol, including:
- Counseling
- Connections to treatment
- Connection to medication
- Overdose Prevention
Client Engagement
We compassionately engage with people living on the street:
- Assess and help people meet immediate needs, such as clothing, shoes, mobility aids, and other essentials
- Connect people with mental health, medical care and substance treatment services
- Arrange longer-term needs for additional care, services, and support
Housing and Support Connections
We provide:
- Information and referrals to available resources
- In-depth assessments to understand what services may be most helpful
- Addressing ongoing challenges related to homelessness
- Offering shelter beds and stabilization rooms when available
- Helping people connect to longer-term housing options
Welcoming spaces for the broader community
- We work collaboratively with City agencies including SF Fire, SFPD, Department of Homelessness, Human Services Agency, Department of Emergency Management and the Mayor's Office to promote safety for all and long-term stability for people experiencing homelessness.
- We partner with many Community Based Organizations including HealthRight 360, RAMS, Heluna Health, Episcopal Community Services, Felton Institute, SF Community Clinic Consortium, Community Forward SF and more.
Mission
The San Francisco Street Team has a dual mission to bring people in crisis on our streets into care, and ensure welcoming public spaces for our broader community. We provide caring and organized responses to people experiencing homelessness. The goal is to support people in crisis, understand their needs, and connect them to long-term help whenever possible.
