What to do
If you have not yet notified your tenants and asked them to sign Appendix B, please do so before going to the next step. (Appendix B of the application is called: List of Participating Tenants.)
Online
- Prepare a pdf copy of the signed Appendix B.
- Complete the application online and submit it along with the pdf copy of the signed Appendix B.
Preparation
- Print the application form in English, Chinese, Filipino, or Spanish. You can also contact Fix Lead SF for a copy (415-252-3882 or fixleadsf@sfdph.org)
- Complete and sign the application form.
- Scan or take a picture of the signed application form and Appendix B (List of Participating Tenants)
