Submit your Fix Lead SF application

Prepare an application packet to fix lead dangers in soil and interior paint

What to know

A complete application packet includes:

  • A completed application form
  • Appendix B of the application form, signed by all tenants

3 ways to send in your application packet:

  • Online
  • Email
  • Mail

What to do

If you have not yet notified your tenants and asked them to sign Appendix B, please do so before going to the next step. 

(Appendix B of the application is called: List of Participating Tenants.)

Online

  1. Prepare a pdf copy of the signed Appendix B.
  2. Complete the application online and submit it along with the pdf copy of the signed Appendix B.

Preparation

  1. Print the application form in English, Chinese, Filipino, or Spanish.  You can also contact Fix Lead SF for a copy (415-252-3882 or fixleadsf@sfdph.org)
  2. Complete and sign the application form.
  3. Scan or take a picture of the signed application form and Appendix B (List of Participating Tenants)

Submitting the application

Email the completed application form and Appendix B to fixleadsf@sfdph.org
