Or call 628-206-7695, and we will schedule an in-home safety assessment with you.
Services
Home Safety Assessments, Minor Home Modifications, & Community Presentations
Home Safety Assessments: Our injury prevention specialist will visit your home to assess your safety needs. If you need safety items, we can provide:
- night lights
- bathmats
- kitchen timers
- grabber tools
Minor Home Modifications: If you meet 100% AMI income eligibility, we can arrange free home safety modifications. Please refer to the referral form for information on income eligibility. For those who are not income-eligible for home safety modifications, we provide low-cost, reputable resources.
Home modifications may include installation of:
- grab bars
- handrails
- shower chairs
- toilet rails
Installations require a signed authorization from the property owner.
Injury Prevention Education Workshops: Email CHIPPS@sfdph.org to schedule a presentation for your residents, activity participants, service providers, and other interested groups.

*Medi-Cal members
Medi-Cal Community Supports provides Home Modifications through all three SF Managed Care Plans (MCP). Health Providers can submit forms to the appropriate SF Managed Care Plan for Home Safety Modification services up to $7,500!
