Request a Home Safety Assessment for yourself or someone else
Or call 628-206-7695, and we will schedule an in-home safety assessment with you.
We cannot accept emailed medical documents.
Services
Home Safety Assessments, Minor Home Modifications, & Community Presentations
Home Safety Assessments: Our injury prevention specialist will visit your home to assess your safety needs. If you need safety items, we can provide:
- night lights
- bathmats
- kitchen timers
- grabber tools
Minor Home Modifications: If you meet 100% AMI income eligibility, we can arrange free home safety modifications. Please refer to the referral form for information on income eligibility. For those who are not income-eligible for home safety modifications, we provide low-cost, reputable resources.
Home modifications may include installation of:
- grab bars
- handrails
- shower chairs
- toilet rails
Installations require a signed authorization from the property owner.
Injury Prevention Education Workshops: Email CHIPPS@sfdph.org to schedule a presentation for your residents, activity participants, service providers, and other interested groups.
