Please bring the following documents to sign up:

1. One Form of Identification:

• U.S. Driver's license
• Permanent Resident Card or Green Card
• Valid Government, Military, State, or Tribal ID
• Passport or Matricula/Consular Card (any country)
• Individual Taxpayer Identification Number (ITIN) Document
• Birth Certificate

2. Current Eligibility Document For One of the Following Programs:

• Medi-Cal/Medicaid
• CalFresh/SNAP
• WIC
• Federal Housing Assistance
