What to do
You can confirm that your PAVE evidence has been received and processed by following the steps below.
Step 1: Download and open the Excel File (Updated monthly)
Step 2: Open and search the Excel file for your staff members' names
Search the excel file for each of your eligible staff
- If the staff member’s name is listed as “NO”—that means that you must submit evidence of PAVE to our office
- If the staff member’s name is listed as “YES”—confirm the information is correct. If corrections are required, communicate this to our office
- If the staff member’s name does not appear—that means you have not submitted an Avatar Account Request form (for a new staff and/or for a newly licensed staff)
Step 3: Submit evidence of PAVE enrollment to BHS Credentialing
Providers must submit a pdf copy of the PAVE Approval Letter to BHS Compliance
Step 4: Resources, information and other guides
- The list eligible provider types that are required to register with PAVE (INSERT Information Notice)
- PAVE instructions, including the contact information for support at DHCS
- Instructions to submit PAVE evidence to BHS Credentialing
- NPI Update Instructions (if needed)
