Apply for Care at Laguna Honda

To receive care at Laguna Honda you must:

• Be a resident of the City and County of San Francisco
• Have a primary diagnosis that is medical (not psychiatric) requiring long-term or short-term skilled nursing or custodial care
• Have existing physical or cognitive limitations that can't be supported at a lower level of care (i.e. Board and Care)
• Be covered by traditional Medicare and/or Medi-Cal (We do not contract with Medicare Advantage or commercial insurance plans. Under limited special circumstances, you may be able to obtain permission for admission from a commercial insurance plan.)

And you must need one of the following:

• Daily skilled nursing (e.g. assistance with two or more activities of daily living (ADLs), IV therapy, wound care, or tube feeding)
• Continuous close monitoring for medical (not psychiatric) conditions (e.g. vital signs monitoring at least every 8 hours by licensed clinical staff)
• Daily supervision (e.g. for dementia-related safety and elopement)

Examples of potential good fits for our level of care:

• Complex wound care, such as stage 3+ pressure ulcers or infected wounds
• Stable tracheostomy care on room air
• Extensive physical support with 2+ activities of daily living such as bathing, dressing, toileting, transfers, and eating.
• Rehabilitation therapy after a hospital stay
• Acute and post-acute rehabilitation for stroke recovery
• Custodial long-term care for people with a disability and/or dementia who need assistance with activities of daily living
